To send in your avails:
Just select and copy all formatted text at the right, in the sidebar of this page and paste it into the form where it says "Paste copied text here", add your avails info and hit 'submit'.

For routing purposes, we want to know the dates you are NOT available, with a list of where (City/State) you'll be on those dates.

Please place an X on open/available weeks.
For booked dates, please enter days booked and the state.
